Case Management

Grassroots NFP LLC Inc. will provide Case Management for participants using the 6 Core Tasks of Case Management.

6. Outcome Evaluation

Food Pantry

Will be available to communities throughout the Quad Cities. It will provide delicious nutritious food. There will be incentivized programs offered to anyone in the community that would like to participate. Program will offer:

• Meal Prep
• Food and Nutrition
• Smart Shopping
• Budgeting
• Advantages of clipping coupons.

Employability Skills

Will provide services that will assist members of the community with skills that will help them access employment.

• Job Seeking
• Resume Building
• Proper Attire
• Motivational Interviewing
• Communication Skills
• Teamwork

Transitional Housing

Grassroots NFP LLC Inc. will provide housing for:

Witness / Victims of Crimes and Domestic Violence: Grassroots NFP LLC Inc will provide immediate shelter. Persons will receive Case Management Services. We will assess the needs of the individuals and services will be rendered accordingly using the model of Safety, Security, Stability, Sustainability, Success.

Re-Entry Residents: Grassroots NFP LLC Inc will provide housing for recently released residents. Participants will be expected to follow the rules set forth by the state. Grassroots will provide structured programs and Case Management services to assist residents to have successful outcomes.

Homeless Veterans: Grassroots will provide housing for Homeless Veterans. Case Management will be offered assisting participants to access well deserved resources.

Community Outreach Services

Grassroots NFP LLC Inc. will provide services for the community if the funds/resources are available.
